Implementing new transfer and admission process and flowchart

As of May 1, 2025, transfers and admissions where one or both facilities are in outbreak will be reviewed and approved by the receiving facility as per the Transfer and Admission Flowchart.

The purpose of this update is to streamline and accelerate the approval process between the transferring and receiving facility.

The new process will be as follows:

  1. The transferring facility will complete the newly updated Transfer and Admission Assessment Form (previously titled the Transfer checklist) and send to the receiving facility for review and approval of transfer or admission requests.
  2. The receiving facility will review the Transfer and Admission Assessment Form and use the new Transfer and Admission Flowchart for the scenarios outlined in section 3.5 of the Recommendations for Outbreak Prevention and Control in Institutions and Congregate Living Settings (PDF).
  3. The receiving facility will provide approval for the transfer or admission to the transferring facility. Transfer or admission should not proceed without approval.
  4. Facilities should consult with Public Health if they encounter a scenario that is not outlined in the flowchart or if there are additional concerns.

Asymptomatic testing prior to the transfer or admission is not required or recommended. If the individual being transferred or admitted is not up to date with vaccinations, they can be offered vaccination by the receiving facility upon transfer.
