*Cette information est seulement disponible en anglais.
To: Local Health System Partners
FOR IMMEDIATE ATTENTION
A transcribing error resulted in Nurse Practitioners not being included in the list of primary care* practitioners in our original registry and advisory alert issued on February 8, 2021. This has now been fixed. We apologize for the error.
Primary Care Registry
In order to meet the demands of the COVID-19 vaccine rollout, Public Health Sudbury & Districts has established a Primary Care Registry to identify local primary care practitioners, who are interested in providing vaccinations to community members across our catchment area.
If you’re a primary care* practitioner and are willing to participate in the upcoming COVID-19 mass and/or mobile clinics led by Public Health Sudbury & Districts, please visit the Primary Care Registry and submit your application. Individuals who register may be contacted by Public Health Sudbury & Districts to schedule participation in clinics pending availability.
*For the purpose of the registry, primary care is defined as health professionals who have the authority to administer the COVID-19 vaccine without medical directive (i.e. MDs, NPs, RNs and RPNs, pharmacists, pharmacist interns, registered pharmacy students, or pharmacy technicians).
Please submit completed registry applications to hr@phsd.ca. If you have any questions, please contact our call centre at 705.522.9200 (toll-free 1.866.522.9200).
Sincerely,
