Wrap-around Supports

The Wrap-around Supports Stream is focused on the collaborative process required to provide social services needed to improve health equity and address the social determinants of health affecting all members of the community. The priorities of the Wrap-around Supports Stream will surround the suite of services required to provide a continuum of care that is tailored to a person’s needs in an organized and integrated manner including housing, social services, and decreasing barriers to accessing services.
