Clinic appointment request form (immunization and tuberculosis testing)

Please use this form to request an appointment in our clinic.

For Tuberculosis Skin Testing (TST):

We offer TST to those who are:

• completing Immigration Medical surveillance
• contacts of active TB cases

We are not offering TST for:

• students
• employment
• volunteer
• medical screening
• screening prior to entry into rehabilitation programs
• persons less than 65 years of age prior to admission to a long-term care or retirement home

Please contact a health care provider or walk-in clinic.

Public Health’s goal is to contact you within 2 business days to provide you with an appointment date and time. You can also contact us during regular business hours to make an appointment.
