FOR IMMEDIATE ATTENTION
In order to meet the demands of the COVID-19 vaccine rollout, Public Health Sudbury & Districts has established a Primary Care Registry to identify local primary care practitioners who are interested in providing vaccinations to community members across our catchment area.
If you a primary care* practitioner and are willing to participate in the upcoming COVID-19 mass and/or mobile clinics led by Public Health Sudbury & Districts, please visit the Primary Care Registry and submit your application. Individuals who register may be contacted by Public Health Sudbury & Districts to schedule participation in clinics pending availability.
*for the purpose of the registry, primary care is defined as health professionals who have the authority to administer the COVID-19 vaccine without medical directive (i.e. MDs, RNs and RPNs, pharmacists, pharmacist interns, registered pharmacy students, or pharmacy technicians)
Please submit completed registry applications to hr@phsd.ca. If you have any questions, please contact our call centre at 705.522.9200 (toll-free 1.866.522.9200).
