OPHS EIDM Wheel 2

A circle separated into four rings. The outer ring contains the four phases of the Sudbury & District Health Unit’s Ontario Public Health Standards annual planning cycle: assess, plan, implement, and evaluate. The second ring contains the seven stages of evidence-informed decision making: define, search, appraise, synthesize, adapt, implement and evaluate. The third ring represents the ongoing process of “evidence tracking”. The center of the circle contains the Sudbury & District Health Unit logo.
