Reporting your COVID-19 vaccination(s) in the provincial database (COVaxON)

If you were vaccinated in Ontario, your records are likely already in the provincial database. You can verify and access your records online by visiting covid-19.ontario.ca/get-proof and log in using your health card number. Your records are in the system if you were vaccinated at a pharmacy, a public health clinic, or by a health care provider in Ontario.

If you have received COVID-19 vaccination outside of Ontario or Canada in the previous 6 months, you may need to submit your proof of vaccination to have your COVID-19 immunization record documented in the provincial database (COVaxON). This ensures optimal protection so that future COVID-19 vaccine doses can be given safely, and at the appropriate time intervals. Out of province or country doses should be recorded for the following groups:

Any other doses will be recorded at the discretion of Public Health. If you need proof of immunization for employment, please show your original vaccine proof to your employer.

Before reporting your vaccinations

Once completed, you will get confirmation that your record was successfully uploaded and will be directed to download your COVID-19 vaccination receipt. This may take several days.
