Reporting your COVID-19 vaccination(s) in the provincial database (COVaxON)
If you were vaccinated in Ontario, your records are likely already in the provincial database. You can verify and access your records online by visiting covid-19.ontario.ca/get-proof and log in using your health card number. Your records are in the system if you were vaccinated at a pharmacy, a public health clinic, or by a health care provider in Ontario.
If you have received COVID-19 vaccination outside of Ontario or Canada in the previous 6 months, you may need to submit your proof of vaccination to have your COVID-19 immunization record documented in the provincial database (COVaxON). This ensures optimal protection so that future COVID-19 vaccine doses can be given safely, and at the appropriate time intervals. Out of province or country doses should be recorded for the following groups:
- Children younger than 5 years of age who have received 1 or more doses outside of Ontario
- Those aged 5 years and older who have received 1 or more doses outside of Ontario within the last 6 months
Any other doses will be recorded at the discretion of Public Health. If you need proof of immunization for employment, please show your original vaccine proof to your employer.
Before reporting your vaccinations
- Please only use this form to submit your records if you currently live in the Sudbury and Manitoulin districts.
- Please complete and submit a separate form for each person.
- Please use the form below to submit your personal information and proof of your vaccination(s). Public Health will input the information into COVaxON. Incomplete forms or those missing information may result in delays in completing the process. If we need additional information, we will contact you directly
- You will need proof, such as the paper or electronic vaccination receipt you received when getting your vaccine(s), with valid QR code, if applicable. If you do not have proof of your vaccination, please contact the health care provider or health agency who gave you your COVID-19 vaccine(s) to get a copy of your record.
- If you received a dose of COVID-19 vaccine in the U.S. and have a paper CDC card, please provide the address and phone number of the location where you received your vaccine.
- If you received vaccines outside of Canada that are not currently offered in Canada, Public Health will review your COVID-19 vaccine records and provide you with advice based on Ontario Ministry of Health. You may need one additional dose of an mRNA vaccine. Use the form below to submit the proof of vaccination information that you currently have, regardless of which vaccines you received.
Once completed, you will get confirmation that your record was successfully uploaded and will be directed to download your COVID-19 vaccination receipt. This may take several days.
