Reporting your COVID-19 vaccination(s) in the provincial database (COVaxON)

If your COVID-19 vaccination records are not in the provincial online database, we encourage you to submit your proof of vaccinations to help public health agencies monitor vaccine coverage. If you were vaccinated in Ontario, your records are likely already in the database. You can verify and access your records online through the provincial system (COVaxON). For example, your records are in the system if you were vaccinated at a pharmacy, a public health clinic, or by a health care provider in Ontario.

If you have received COVID-19 vaccination outside of Ontario or Canada you will need to submit your proof of vaccination to have your COVID-19 immunization record documented in the provincial system (COVaxON) to obtain the enhanced vaccination certificate with QR code.

Proof of vaccination

Once your vaccination records (for example, immunization record or proof of vaccination certificate (PVC)) are added to the provincial online database, you will be able to access a digital enhanced vaccination record with a QR code.

If you currently live in Sudbury and districts and your vaccination record is not recorded in the provincial database (COVaxON), Public Health Sudbury & Districts can add your record to COVaxON for you.
Please use the form below to submit your personal information and proof of your vaccination(s). Public Health will input the information into COVaxON. If we need additional information from you to complete your record, we will contact you directly.
If you received vaccines outside of Canada that are not currently offered in Canada, Public Health will review your COVID-19 vaccine records and provide you with advice based on the Ontario Ministry of Health. If you received a vaccine that is not offered in Canada, you may need one additional dose of an mRNA vaccine. Use the form below to submit the proof of vaccination information that you currently have, regardless of which vaccines you received.
Please complete and submit a separate form for each person.
Once completed, you will get confirmation that your record was successfully uploaded and will be directed to download your COVID-19 vaccination receipt. This may take several days.

Is your record already in COVaxON?

Your records will not be in the provincial database if you were vaccinated outside of Ontario or Canada. Use the form below to submit your records.
Some people vaccinated in Ontario received vaccine from what is referred to as “non-Ontario stock”. This refers to vaccine that was not allocated to Ontario or recorded in the provincial vaccine management system. In general, these doses were given in institutions such as correctional facilities, to military groups, or at embassies. If you received one or two doses of vaccine from the non-Ontario stock, use the form below to submit your records.
If you were vaccinated in Ontario, your records are likely already in the system. You can verify and access your records online through the by visiting covid-19.ontario.ca/get-proof and log in using your health card number. If you are unable to access your records, please Public Health Sudbury and Districts 705.522.9200 (toll-free: 1.866.522.9200), between 8:30 a.m. and 4:30 p.m., Monday to Friday.

What you will need to report your vaccinations

To report your vaccinations, you will need proof, such as the paper or electronic vaccination receipt you received when getting your vaccine(s). If you do not have proof of your vaccination, please contact the health care provider or health agency who gave you your COVID-19 vaccine(s) to get a copy of your record.
